
We, the Clinicians for Free Iran Network, express profound alarm at the rapidly escalating violence and grave human rights violations unfolding across Iran. Evidence from credible human rights organizations confirms that the Iranian state is actively preparing and executing mass violence against its own population.
The nationwide protests that began on 28 December 2025, driven by economic collapse and demands for fundamental political and social rights have been met with lethal, coordinated repression. Security forces have used live ammunition, mass beatings, arbitrary arrests, and battlefield-style tactics against largely peaceful civilians across multiple provinces. (Human Rights Watch)
Civil society monitors report hundreds of civilians killed, including children, with hundreds more critically injured. Thousands have been arbitrarily detained and now face a high risk of torture, enforced disappearance, or extrajudicial execution. (AP News)
At the same time, the regime has imposed a near-total internet and communications blackout, deliberately concealing crimes, preventing emergency response, silence witnesses, and isolating families from loved ones. (AP News)
State Responsibility and Accountability
These crimes are not isolated abuse. They are the direct result of state policy and command decisions made at the highest levels of power Supreme Leader Ali Khamenei and President Masoud Pezeshkian, who, despite being purportedly a physician, presides over and enables the violent assault on civilians, hospitals, and medical professionals.
We are particularly alarmed that:
- Hospitals and medical facilities are now besieged by regime security forces, blocking access to care, arresting or kidnapping the injured, and intimidating healthcare workers turning places of healing into instruments of repression. (Human Rights Watch)

Call for Immediate International Action
We urgently call on the United Nations, World Health Organization (WHO), Office of the High Commissioner for Human Rights, International Committee of the Red Cross/Red Crescent, and the entire international community to act immediately and decisively:
- Stop the Massacre and Protect Civilians
- Condemn the use of lethal force and demand an immediate halt to all violence.
- Demand the unconditional release of all individuals detained for exercising fundamental rights.
- Protect Healthcare and Medical Neutrality
- End the siege and militarization of hospitals immediately.
- Ensure no injured person is arrested, kidnapped, or denied life-saving care.
- Protect healthcare workers from persecution and intimidation.
- Restore Communications and Transparency
- End the internet blackout immediately.
- Allow unrestricted access for independent media and human rights monitors.
- Activate Accountability Mechanisms
- Launch independent UN investigations into unlawful killings, torture, enforced disappearances, and crimes against humanity.
- Pursue legal accountability through universal jurisdiction where applicable.
- Mobilize Emergency Humanitarian and Health Support
- Deploy urgent trauma care, medical supplies, and psychological support.
- WHO must coordinate an immediate crisis health response.
The situation in Iran is critical and deteriorating by the hour. Silence and delay will enable atrocities.
